If your child is scheduled to attend Eastlake Middle School next year (students living in the 91914 or 91915 zip code), your student had an orientation with the ELM administration team today, Friday, February 21st, 2025. Students were able to meet their new counselors and learn about campus life for next year.

The next step each student needs to take is to start completing the online process of selecting their initial courses and pre-registering for school. Official registration information will come later from Eastlake Middle School. For now, please:

- Upload immunization records and over-the counter medication form to Eastlake Middle’s nurse: Click Here

- Complete affidavit form: Click Here
- Upload completed affidavit form and proof of residency (Utility bill, lease agreement, or pay stub dated within the last 60 days): Click Here
- Complete the 7th Grade Course Selection Google Form. All incoming 7th-grade students must submit this form to select elective classes and indicate any specialized programs such as DLI, an IEP, or 504 Plan: CLICK HERE

- Optional: Complete the Compacted (Accelerated) Integrated Math 7 Google Form. Only complete if your student wants to enroll in the Compacted (Accelerated) Math Program: Click Here
